JSON string of document metadata. Required: patient_id, document_id, type (CLINICAL|CONSENT), title (max 128 chars), mime_type, creation_time (RFC3339), format_code, type_code (LOINC, e.g. 11502-2), class_code (LOINC), practice_setting_code. Optional: confidentiality_code (default: N), healthcare_facility_type_code (default: 394777002), service_start_time (RFC3339), service_stop_time (RFC3339). Note: patient_id must already exist in Particle MPI.
