If only we had known that Hart couldn't be seen by the cardiologist for 2 months... or If only we knew when the cardiology consult took place..., we could have reached out for the note and collaborated proactively.

Particle offers Referral Management as part of our Signal solution. This product enables our customers to enroll a patient (or a cohort of patients) that they have referred to another provider outside of their own provider group or network.

Once the customer shares the information about the referral, Particle Health will then start monitoring the health information networks to see when new data has come in from that organization and for that patient. Your team will receive an alert when the referral encounter takes place and you will be passed any net new data that was generated by that encounter.

You will also know when a patient does not follow up on a referral so you can take action accordingly - perhaps the wait list was too long, or the patient forgot to schedule it, or the patient did not show for the scheduled appointment because of transportation issues.

In Hart's case, Signal would have given you visibility on when (or if) Hart followed through with his cardiology visit, and would have allowed your care team to proactively reach out to the practice to obtain the consult note sooner, at least ahead of the follow up visit in order to support continuity of care and ensure Hart's care doesn't fall through the cracks.
