Particle connects to Manifest MedEx a state information exchange for California.
Background
Value-based care (VBC) organizations face significant challenges in identifying emergent patient encounters—such as ED visits, urgent care visits, and hospitalizations—in time to intervene within the critical 72-hour post-discharge window. This delay hampers their ability to reduce readmission risks and manage care costs for their highest-risk patients.
Currently, admit, discharge, and transfer (ADT) notifications—standardized messages exchanged between EMRs in HL7 format—serve as the primary mechanism for tracking emergent encounters. However, ADT solutions have notable limitations. They often lack comprehensive facility coverage, forcing organizations to use multiple systems, which drives up costs. Even then, providers rarely receive complete notifications from all the facilities their patients may visit. Furthermore, ADT messages are difficult to interpret, and without accompanying encounter data (which providers seldom have), they offer little actionable insight to improve patient outcomes.
Overview
Particle is integrating with Manifest MedEx, a California state HIE.
By integrating with Manifest MedEx, Particle Signal aims to deliver a superior alternative to standalone ADTs for California patients. By ingesting ADT data, extracting critical clinical context, and enhancing it with analytics, Particle Signal will empower providers with actionable insights. The foundation of this solution is comprehensive ADT data ingestion, enabling better visibility and more effective care interventions.
In addition to providing critical ADT coverage, the Manifest MedEx integration will enable our California customers and prospects to comply with AB 133 regulatory requirements for data sharing with the California state HIE.
It’s important to note that this integration does not expand Particle's EMR data coverage, as we already access the same data through our existing eHealthExchange integration.
What is specifically possible with the Manifest MedEx integration via Particle?
- Particle's customers can upload a roster of all California patients to be monitored for ADTs
- Customers will receive notifications when any of their monitored patients have an ADT event in California.
- When customers send bi-directionality documents to Particle to satisfy the national networks' requirements, Particle will forward those documents to Manifest MedEx to meet the California regulations.
