Particle's Platform unifies and consolidates clinical records from thousands of data sources. We share discharge summaries, condition and medication lists, imaging reports and lab values, vital signs, social history and more outlined below. You can expect to see historical records for as far back as the networks have them.

Health Information Exchanges

Electronic health information exchange (HIE) allows care teams and patients to appropriately access and securely share a patient’s medical information electronically. Particle’s Platform uses three nationwide healthcare data exchange networks, Carequality, Commonwell, and eHealth Exchange as well Healthix, and Manifest Medex the public HIEs for downstate New York (including New York City and Long Island) and California, respectively. The networks vary in terms of their capabilities and requirements; Particle manages all of this complexity on behalf of our clients.

Particle is an active member of these networks and participates in committees related to standards definition, implementation, and deployment.

Surescripts

Ever wonder if your patient filled a medication you prescribed? Or, whether they have a pattern of missing or late medication fills? Medication data sent through the EHRs is notoriously messy and incomplete. Medication statuses, start and end dates are often not populated, and not always accurate. Particle Health's partnership with Surescripts enables us to integrate robust medication fill data with clinical data to create a more comprehensive and actionable view of a patient's health. This holistic approach enables healthcare providers to gain deeper insights into patient care by connecting medication adherence patterns with diagnoses, treatment plans, and clinical outcomes.
